Job Summary:
We are seeking a skilled and compassionate Nurse Practitioner to join our Home Hospice Care Team.
As a Nurse Practitioner, you will provide high-quality care to patients across the lifespan, diagnose and treat various medical conditions, and collaborate with other healthcare professionals to ensure comprehensive patient care.
Responsibilities:
- Conduct comprehensive physical admission assessments and Medicare recertification assessments, including vital signs and medical history
- Order and interpret diagnostic tests and screenings
- Develop and implement treatment plans for acute and chronic illnesses
- Prescribe medications and monitor patient response
- Provide patient education on disease prevention, management, and treatment options
- Collaborate with physicians and other healthcare professionals to coordinate patient care
- Perform procedures such as suturing, wound care, and minor surgical interventions
- Maintain accurate and up-to-date medical records
Experience:
- Minimum of 2 years of experience as a Nurse Practitioner in a clinical setting
- Proficiency in performing vital signs assessment, ventilator care, infection control measures, and diagnostic evaluations
- Knowledge of ICD-10 coding system for accurate documentation and billing purposes
- Familiarity with psychopharmacology for mental health treatment
- Experience working in assisted living facilities is a plus
Qualifications:
- Valid Nurse Practitioner license in the state of practice
- Master's degree in Nursing with specialization as a Nurse Practitioner
- Certification as a Nurse Practitioner from an accredited program
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
