RN Case Manager

_Hospice Care Team_ is looking to hire a Registered Nurse Case Manager to assist with our hospice patient visits in Brazoria County! Working on a full-time basis, the RNCM will work with a team of healthcare providers to ensure our home health patients are receiving quality care.
*Responsibilities:* * Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings.
* Coordinating, assessing, and implementing patients' care.
* Providing accurate and timely documentation of patient services to reflect plans of care.
* Flexibility to be available days, evenings and weekends.
