We are looking for an outstanding *Registered Nurse (RN)* to join our team in *League City, TX*.

*Core Responsibilities:*
* Conduct assessments of incoming patients
* Manage your own case load and conduct visits where patient resides
* Provide symptom management and oversee patient’s plan or care while collaborating with Hospice team members
* Participate in anon-call rotation for evenings and weekends
* Have knowledge and understanding of all services hospice provides and keep patient and family informed of all benefits available
*Day to Day Activities:*
* Educate patient and family on disease progression, make visits, care coordination and management of patient and family needs related to Hospice diagnosis
* Deliver outstanding bedside care
* Foster relationships with your patients and families to make a difference in their greatest time of need
*Who the Job reports to:*
* Director of Clinical Services
*Requirements/ Qualifications:*
* Valid Registered Nurse (RN) license
* 1-2 years of nursing experience (preferred)
* Driver’s license, insurance and vehicle
* Utmost compassion and integrity
