Documentation of patient’s condition and pointing out significant changes, to be reported to the physician
Collaboration with our partners and sister companies.
Collaboration with our interdisciplinary team such as; Social Worker, Physicians, CNA's and Hospice Chaplain.
Requirements
Texas or Compact Nursing License
On call
Strong organizational and self-management skills
Valid driver’s license
