Required:
• Graduation from an accredited nursing program
• Licensed to practice as an LVN in the State of Texas
• New Grad with 0 – 2 years of clinical experience
• Must have knowledge of adult and child learning principles, ambulatory nursing practices and procedures, and community resources
• Current CPR Certification
• Ability for spur of the moment transportation without relying on organization vehicles
• Must possess or ability to readily obtain a valid driver’s license issued by the State of Texas for the type of vehicle or equipment operated
• Department of Motor Vehicle check may be required, if applicable
• Customer Service Oriented
• Excellent written and verbal communication skills
• Excellent Computer Skills MS Office
• Must be willing and able to work evening and weekend hours if necessary
• Must be in compliance with GCHD Immunizations policy
• Must be in compliance with ICS training requirements
• Must pass criminal background check and drug/alcohol screening
An equivalent combination of education and work experience which appropriately demonstrates the knowledge, skills and abilities to perform the above described essential functions will be considered when hiring for this role.
Traditional Duties:
• Assists providers in completing all medical documentation needed to provide health care to patients within the electronic health record
• Provides direct and indirect nursing care for patients of all ages
• Provides nursing assessments and triage activities under supervision
• Provides customer-focused clinical care
• Assist Medical Assistants and Providers with all aspects of providing care to the patient
• Assists with procedures, prepares patients for, and assists providers with exams, performs and documents screening tests; demonstrates age-appropriate modifications as related to measurements and screenings
• Accurately documents all nursing assessments and care, patient education, follow-up, treatment plans, etc.
in the electronic health record
• Performs approved POCT tests in the clinic setting as ordered by providers and documents results
• Performs daily, weekly, monthly, and quarterly maintenance when needed on equipment
• Performs quality assurance and quality control activities such as vaccine and medication inventories
• Safely and properly administers medications, immunizations, and treatments per provider orders and/or standing delegation orders
• Refilling approved medication refills per standing delegation orders
• Triage and process messages/task/communications and/or calls from patients, front office staff, and contact center
• Review Immunization Records in EHR and ImmTrac2 then reconcile records and add historical vaccines
• Perform Nurse Only visits
• Perform wound care
• Performs prior authorizations
• Assist with supply inventory
• Completes patient forms when necessary
• Provides reports to other departments
• Performs medications disposal for expired medications
• May assist CHW Nursing Director by presenting education, training or in-service material to nursing personnel
• Assesses patient education readiness and modifies patient education according to patient and family needs
• Provides orientation, education and supervision of inexperienced staff nurses, students, auxiliary workers and volunteers
• May serve on clinical teams
• Understands and follows GCHD/CHW Infection Control policies, plans, procedures and guidelines related to program area requirements
• Properly disposes of biohazard waste
• Adhere to job specific competencies within established departmental deadlines
• Assist with Charge Nurse duties as delegated
• Performs other duties as assigned by supervisor
