Serves Brazoria and Galveston County, TX FLSA Status: Exempt Working Status: Full-Time with Benefits Summary: To provide therapeutic services to individuals, groups, and families enrolled in Youth Behavioral Health Services from Brazoria and Galveston Counties.
Provide clinical consultation to staff as needed.
Serve as a member of the Youth Behavioral Health Services Interdisciplinary Treatment Team for both counties.
Conduct diagnostic evaluations for youth as a member of the Youth Behavioral Health Services Team (as needed).
Diagnostic evaluations and therapy are to be provided at the Angleton and Texas City locations.
Manage assigned caseload inclusive of providing input on person-centered recovery plans and 90 reviews.

Job Duties: (To be performed with or without accommodation as based on ADA requirements) The Therapist - YBHS will be responsible for the following essential Job Duties: Will complete LPHA assessment necessary to determine eligibility for admission (inclusive of priority population).
Complete the required diagnosis form indicating all five diagnostic categories.
Accurately complete all LPHA Center required documentation necessary for the admission process.
Assure all required LPHA documentation is printed, signed, and filed in respective records.
Conducts service delivery in accordance with Youth Behavioral Health Service Standards as defined by state and local authorities.
Assures services are not provided on assigned youth should the recovery plan and/or CANS not be valid Identifies, arranges, advocates, and collaborates with other agencies to ensure an effective planned referral, discharge, or transfer of services.

).
Provide crisis services to consumers and walk-ins to the clinic as needed.
Conducts initial screenings by telephone or in-person and provides crisis intervention and referrals as appropriate.
Face-to-face screenings should on average be completed within 30 minutes.
Reviews Recovery Plans for youth not in medication services through GCC for accuracy and oversight of service provision as LPHA.
Conducts the Children's Need & Strengths Assessment every three months or as clinically indicated for individuals as required & assigned.
Responsible for ensuring the completion of the initial & annual CANS recertification in accordance with the Praed Foundation & HHSC performance contract requirements.
A lapse/expiration of certification will result in termination of employment.
Exception: If certification lapse/expiration is reported to the supervisor within 24 hours of the expiration date, the employee will be granted the opportunity to recertify by the close of the following business day.
Any lapse/expiration greater than 24 hours will result in termination of employment.
Additionally, all certified CANS users are required to participate in semi-annual quality assurance training activity conducted by the identified CANS Super User.
Provides Case Management services as needed with youth on assigned caseloads or as a backup for TRR case managers.
Serve as a member of the interdisciplinary treatment team providing group and/or individual, family therapy services as needed and authorized by a valid Recovery Plan.
Utilize Smart Care reports in order to consistently monitor and complete required documentation as indicated by program and contract standards.
(Including minimum time plus staff time reports.
) All documentation is complete, accurate, and clear as required by GCC Consumer Records Procedure within 2 business days and filed in the electronic health record.
Participate in team meetings as requested to provide clinical continuity.
Consistently practice the provision of follow-up and communication related to all services provided.
This is inclusive of other agencies, families, and teams.
Meets or exceeds the established performance requirement for the entire caseload each month.
Meets all productivity measures determined by leadership staff.
Conduct service delivery in accordance with Youth Behavioral Health Services standards as defined by the agency, agency collaboration, and program.
Assure services are not provided to assigned youth should the treatment plan not be valid.
Effectively communicate with the assigned team, collaborative agency members, and leadership staff regarding service delivery, continuity of care, and employment/administrative issues.
Report personnel leave; flex time and emergency leave to the assigned supervisor.
Implements 'Fish' philosophy in approach to assigned work duties.
Emphasizes principles of philosophy when collaborating with co-workers and during service provision.
Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop, and continue our community partnerships as they relate to the position.

Supervision: Receives direct supervision from Program Manager-YBHS Required Education: Minimum of a Master's Degree with an LCSW, LMFT, or an LPC.
Required Licensure/Certifications: LCSW, LMFT, or an LPC.
Required Experience: N/A Preferred Experience: One to two years of related Mental Health experience is preferred Abilities/Traits: Detailed knowledge of mental disorders and child development Self-motivated with good time management skills Effective interpersonal skills Ability to establish cooperative relationships with youth and their families Mature problem-solving and self-directed approach to daily work Ability to accept responsibility and exercise independent judgment in making decisions Ability to express self clearly and effectively, orally and in writing.
